Potential Requirements

This application may require the following documents as electronic attachments. Accepted files: JPG, JPEG, PNG, PDF, DOC, DOCX

  1. 501©3 Certificate (if applying as a Non-Profit Agency)
  2. Event Layout / Site Plan: A plan denoting buildings, parking spaces, and placement of signs and/or tents.
  3. Vendor Details: A detailed listing of vendor names including their addresses, phone numbers, and type of service of any person(s) to whom you have an agreement/contract for any service they will provide.
  4. License to Distribute Alcohol (if alcohol will be served during your event)
  5. Tent Permit(s) (if using a tent larger than 10 x 10)
  6. General Liability Insurance Certificate:

      The Certificate Holder is an Additional Insured as granted by the General Liability (and/or Automobile Liability) policy for liability arising from the premises use/operation of the Named Insured.
